My friend managed to reach champions, complete the achievement and collect the gems with only 3193 cups.

I believe he won a defense to cross the 3200 mark and then fell back down before attacking? Seems like I remember defensive wins not updating the trophy record.

If you gain trophies on defense, they don't appear in your stats until you log back into the game.
He would have gotten to Champ in defense, and then was attacked again, and lost those trophies, before returning to the game again.

That's right.
When I first hit champ I did it with a successful defense, but lost a defense thereafter. The game considered I hit champ, but my profile did not consider the trophy count at the highest point.
Way back I posted a topic in Ideas & Feature requests to try change it but never saw the light of day.
