I love the opt in and opt out shields, they make it much easier to look after a clan wars situation. But what happens when someone just goes inactive ? Or what about that new guy with a rushed base that you dont want in wars until he has fixed up a few things ? Do you just kick him ?
Sure a leader and Co should know who in the clan are active and who are not. But that is not always possible.
Can the Opt in and out shields be triggered by a Clan leader as well as the individual ?

Even if everyone has their shield green (opt in) the leader can still de-select players that he doesn't see fit for the clanwar. The opt in/opt out option is just an indicator for the leaders, but in the end it all comes down to clan management and member quality knowledge.
Yes, but not every Co does. And if you are doing b2b wars you need to share the load.
