"Total heal" has similar meaning as "total damage" in Lightning or Poison spells.
Heal spell consists of 40 individual heal pulses over the span of 12 seconds (so one pulse every 0.3 second).
Poison consists of 45 damage pulses over the span of 15 seconds (so one pulse ever 0.33 second).
Lightning consists of 6 lightning strikes over the span of 2.4 seconds (so one strike every 0.4 second).
All three spells work similarly, they only vary in effect (damage or heal), area covered and time in between each time the effect is applied.
Dropping a heal spell where there's no friendly troops is similar to dropping a lightning where there's nothing to hit. The heal effect is still applied, and just "misses" since there's no targets to heal.
When a troop enters or exits the heal area is the same as a troop walking into the area of lightning spell. Before entering the area heal pulses "miss" the troop and once the troops enters they start "hitting", which results in the troop receiving a partial amount of the "total heal".
It's like stepping in and out of the rain. If you step out into the rain for 2 seconds only you will get about half as wet than if you step out for 4 seconds.
