GO OFFLINE FOR A FEW HOURS!!! If that doesn't work, stay offline all day and check it the next day.
I'll survey it if necessary, but I'm guessing (whether the people who aren't getting headstones admit to it or not), that everyone who has complained about not receiving a headstone, has been online for inexplicably long periods of time.
Here are the Details of how I came to this conclusion:
How do i know for sure. I won't guarantee it, but I tested it on my two accounts. When the update first came out, neither of my accounts was getting obstacles, but that was because I was on them both a lot. I started some upgrades on my second account which were going to take a few days, so I went offline for an extended period of time, and when I checked my account again, I had several obstacles and a.... yes... a Headstone.
I was still playing pretty aggressively on my primary account, and that one hadn't received more than 1 obstacle since the morning of the Halloween update. I continued to play aggressively until I started several upgrades and went offline for a longer period of time. Honestly I noticed it by the 4th hour of being offline, and yes.... I received a Headstone as well as another obstacle.

I'm adding this to the bottom for those who are having a hard time grasping this. If you are stuck on believing something else, sorry I don't perform miracles, I'm just a very observant person and hopefully you are as well...
A lot of dumb dumbs. Ok 1 more time, if you get it great, if you don't, oh well. I'll even label and number this off for all those with learning disabilities.
Fact #1 ...... Everything that spawns on it's own is an obstacle
Fact #2 ...... Obstacles have the "opportunity" to appear every 8 hours
Fact #3 ...... If you are NOT receiving obstacles every 8 hours, it is because you were online when the opportunity came around
* this is the one dumb dumbs are having a hard time grasping, but several people have already said that it is true...
* Because people who stay online all the time don't receive as many obstacles as people who are offline most of the time
Fact #4 ...... The headstone is an OBSTACLE
Fact #5 (the sum of all things) ...... Put your critical thinking hat on now. Put together "Fact #2" and "Fact #3" with "Fact #4".
If you have semi decent critical thinking skills, you might understand that you are not guaranteed a head stone next time an obstacle appears, but if you are online when the "opportunity" to receive one comes up, you are going to miss the "opportunity" for another 8 hours, and if you are always online, you might always miss the opportunity.
I'm sorry, if you don't get it, maybe someone who does get it can dumb it down a little more like a picture book or something. Just don't get mad because what I'm explaining goes against what your best friend, clan leader, or role model told you.
I've been validating it, the way things should be validated, I was online for several days as much as anyone could be online, and I wasn't getting obstacles. I went offline first with one of my accounts, and guess what I got a bunch of obstacles (including a headstone). My primary account, still no obstacles, but then I went offline with that one too, and guess what.... I got a bunch of obstacles including a head stone and a gem box.
Get it? Test the theory if you believe otherwise, than come back and talk.
