You almost have to use rage spells with balloonion, but I also like to carry a heal spell with me. It helps when dealing with WTs and even xbows if they aren't in range of infernos, and it's also very helpful against TH 9 opponents. That being said, heal spells are a matter of personal preference, not necessity.
I actually carry a lightning heal and 2 rage
Hit the nail on the head... for me, one Heal is especially useful when farming, not just as Zach descibes, but also just because it brews so much quicker which is great when you are attacking every 30-40 minutes. Then one lightning, the rest rage (maybe one freeze).
For cup-hunting / war, I do not necessarily use a Heal; if you have enough balloons (say 24+), likely you'll just want all the rage you can get (maybe one freeze, for multi-inferno), to speed them on their way to the core, and help the minions once they get there...
I recommend heal because balloonions are super OP if they keep regenerating health, they will just glide right over bases.
Voted rage because I mostly carry rage for loons.
As TH9 I carried 3 rage + 1 extra. Either a lightning for pesky CCs that cannot be lured efficiently, or a heal to help them live longer - which as Zach said is quite handy vs TH9 or TH10 without Infernos.
I always carry rage spells with loons & will sometimes carry a heal.
As a TH9 I usually do 2 rage, 1 heal, and 1 lightning or 2 rage and 2 heal.
