You'll find more info in bugs and problems.
#1 make sure you account is linked to Game Center or you will lose it.
Then log into the Game Center with new ipad and your village will be there. After that I think you'll need to do a back up of your iphone via itunes then factory reset the phone to clear it of old CoC info.
