Then I decided to reset the phone because he started to work poorly (slow). Returned to the factory settings.

First off, thanks to AnonAngent! This thread is a great idea and seems to have helped those who've lost access to their respective villages and provided some much-needed additional info. And kudos to Twister for adding insight as to the process for the player support team in dealing with the numerous requests on this issue.
I also lost access to my village about two weeks ago while trying to update CoC and didn't have it linked to Google+ at the time. Here's the details for addition to the list being compiled:
Village name: Thorin
Level: 42
Clan: New Bahamas 242
Town hall level: 6
I reported through the in-game tool on December 5th (local time) - I think would have been December 6th in GMT time zone and my request/ticket # is 1350278 (follow-up requests # 1353698 and 1355262 just to provide additional info Supercell asks for such as name and level of 'new' village i.e. the one I can currently access after re-install).
So patiently waiting for player support to get to my ticket number in the queue and eagerly looking forward to getting back my village! And a suggestion for the Supercell development team, similar to what AnonAgent already mentioned: why not add the linking of Google+ to the tutorial a player goes through when they start a new village in CoC? That would help to ensure that players complete this crucial step before getting into building up their village. And this benefits both the players and Supercell - fewer frustrated and discouraged players submitting fewer requests to support agents! More players happily clashing, purchasing gems and telling everyone how much fun they're having playing CoC and inviting people to their respective clans! A win all around!
I've only been playing since October of this year, and call me crazy if you want to, but from what I've seen SC puts a lot of time and thought into what they do and CoC is an excellent offering in the social gaming/clan gaming genre. So constructive feedback can only help to improve the gaming experience for all.
